Student Safety Insurance
π 1. Application Period

Current Students: Must file a claim within 2 years from the date of the incident. (Students on suspension must have paid insurance fees to be eligible.)

Graduates: Incidents occurring before July 31st of the graduating semester are also eligible for claims within 2 years.
π 2. Eligibility
-
Officially enrolled students of National Tsing Hua University (including Teacher Training Center interns) who have paid the current semester’s group insurance fee.

Hospitalization or surgery due to illness (inpatient procedures only; emergency room visits without hospitalization do not qualify).
-
Accident-related emergency and outpatient treatments are covered up to NT$5,000; hospitalization benefit NT$750/day; surgical reimbursement up to NT$30,000.

Weekdays 8:00 AM – 5:00 PM, at Shui-Mu Student Center 2F, Student Assistance Office.
4. Death Claim Application
Required documents:

Claim application form (signed and stamped by the beneficiary)

Original autopsy certificate from the District Prosecutors’ Office or hospital-issued death certificate

Original household registration transcript for the entire household (must indicate student’s death)

Original personal household registration transcript (must be deregistered)

Copy of Student ID (both sides)

Copy of beneficiary’s bank account front page

Copies of both parents' ID cards (front and back)
-
Medical diagnosis certificate and receipt (if any medical treatment before death)

X-ray disc (if applicable)
-
Accident report issued by traffic police (for car accidents)

Consent form signed by all legal heirs (children aged 7 and above must also sign)
π‘οΈ 5. Insurance Coverage During Suspension

Students on suspension who retain student status may apply for continued insurance coverage within the first three weeks of the semester.

Must pay insurance premium before the semester’s registration deadline.
-
Failure to pay on time is considered waiver of coverage.

Insurance fee will not be refunded for mid-semester suspension; coverage remains valid for that semester.
β‘ 6. Important Notes

Each student receives an NT$50 subsidy per semester from the university; remaining insurance fees are self-paid.

Group insurance covers hospitalization, surgery, accidents, major burns, first cancer diagnosis, disability, and death claims.

For club activities or off-campus fieldwork, students or departments must arrange their own accident insurance with a chosen provider.
